William Duggan

Dr. Duggan is Board Certified in Family Medicine. He was accepted by the prestigious Royal College of Surgeons in Ireland at age 17 for a six-year program in medicine. Afterwards, he emigrated to the United States for a microsurgery fellowship at the Cleveland Clinic. During this fellowship, he realized surgery wasn’t the kind of medicine he wanted to practice. Instead, he accepted a Cleveland Clinic family medicine residency and also acted as a “shadow” fellow in sports medicine, working with physicians for the Cleveland Browns and Cleveland Cavaliers, as well as area college athletes. Dr. Duggan emphasizes openness and accessibility in his practice and views the doctor-patient relationship as a collaborative agreement or a cooperative plan. “It’s easy to become disease focused in medicine, but people are defined by more than just their health conditions,” says Dr. Duggan. “Yes, I know how to treat illness, but I am a gateway to health information. I like to discuss that information with my patients. Primary care has drifted away from what it used to be, and I want to help make it relational again.” Dr. Duggan joined Community Health Care, Inc. in 2022.

Linda Keith

Dr Linda Keith, DO joined Community Health Care after completing her Family Medicine residency at Summa Barberton Hospital in 2002. She is currently board certified in Family Medicine. She is also a member of the American Academy of Family Practitioners, the American Osteopathic Association and the American College of Osteopathic Family Practitioners. Dr Keith also precepts Physician Assistant students associated with the University of Mount Union.

Dr Keith enjoys seeing patients of all ages and treats a wide variety of illnesses and conditions. She believes Family Medicine is about relationships and caring for entire families across multiple generations.

In her free time, Dr Keith enjoys fitness, traveling with her family, working in the garden and playing with her two Labrador retrievers. She also has been PADI certified and loves to scuba dive in the Caribbean Sea with her husband.

Dr Keith is excited to get to know you. She is surrounded by an excellent support team consisting of medical assistants, registered nurse care managers and behavioral health specialists to help you achieve your physical and mental health goals. She joined Community Health Care, Inc. in 2002.

Angela Hissner

Angela Hissner is a Board Certified Family Nurse Practitioner. She received a Master of Science in Nursing from Malone University. She has clinical expertise in diagnosing and treating health conditions with an added emphasis on disease prevention and wellness care. In her free time, Angela enjoys running and she has completed 5 marathons. Angela joined Community Health Care, Inc. in 2017.

Leila Abraksia

Leila Abraksia is a board-certified Family Nurse Practitioner passionate about providing compassionate, evidence-based care to individuals and families across the lifespan. She began her healthcare career working in clinical trial research after earning her Bachelor of Science in Nutrition from The Ohio State University and completed her Bachelor of Science in Nursing through an accelerated program from Cleveland State. She spent nearly five years in the cardiac ICU at Cleveland Clinic before joining Community Healthcare in 2025.

Leila also has a Master of Public Health, which enriches her holistic approach to patient care, emphasizing preventive health. She believes that great healthcare starts with connection — taking the time to listen, understand your story, and work together to create a plan that fits your life and goals.

She enjoys spending her free time with her two dogs, running, cooking/baking, traveling, and making memories with her fiancée, family and friends.

Sara Tokie

Sara Tokie is a Board Certified Family Nurse Practitioner. She graduated with her Masters from Chamberlain University in 2019. She treats patients of all ages with a passion for diabetes and women’s health. She lives in North Canton with her husband and two children. In her free time, she enjoys gardening and doing crafts. Sara joined Community Health Care, Inc in 2023.
