This practice opened in April, 1997. The office now located at 1302 W. Main St. in Louisville, just about a mile toward downtown Louisville from our old location at 5862 Louisville St. NW. Patients are drawn to this office from Louisville, Hartville, Malvern, Minerva, Waynesburg, Canton, East Canton, Alliance, and the surrounding areas.

The office hours are: Monday and Tuesday 8:30am-5pm, Wednesday 8:30am-1pm, Thursday 8:30am-8pm, Friday 8:30am-5pm, and Saturday 8:30am-12noon.

The practice admits to Mercy Medical Center and Aultman Hospital, both in Canton. The office number is (330) 875-5544. For those living out of the area (Minerva, Malvern, etc.) the number is (330) 455-0999. The office manager is Barb Webster.

PROVIDERS:
This practice is served by William Lago, M.D. He is a graduate of the Northeastern Ohio Universities College of Medicine, and did his family practice residency at Aultman Hospital in Canton. He is board certified in family practice with the American Board of Family Practice, and is a member of the American Academy of Family Practice. He has been the team physician for East Canton High School for many years, and is also the medical director of the McKinley Health Care Center.    Community Health Care, Inc. is pleased to welcome Melody Alazaus, Family Nurse Practitioner, to our office at Community Health Care Louisville in Louisville, Ohio as of November, 2005. Melody is a graduate of the masters of nursing program at Case Western Reserve University in Cleveland,  and is a certified family nurse practitioner. She is able to care for patients of all ages.
