Community Health Care Diabetes Services

Our goal at Community Health Care is to provide a comprehensive, individualized education program about diabetes with an emphasis on self-management skills.

We believe that being educated about diabetes is vital to successful treatment of diabetes. The more patients know about managing their diabetes, the more active they can be in their own care.

Your physician and a specially trained nurse practitioner educator will provide practical information the day-to-day management of diabetes. Continuing care will focus on support, ongoing education, and prevention of long-term complications.
