Our goal at Community Health Care
is to provide a comprehensive, individualized education program
about diabetes with an emphasis on self-management skills.
We believe that being educated
about diabetes is vital to successful treatment of diabetes. The
more patients know about managing their diabetes, the more active

Your physician and a specially
trained nurse practitioner educator will provide practical information the
day-to-day management of diabetes. Continuing care will focus on
support, ongoing education, and prevention of long-term
